Deadline for form submissions - May 15th
On this page, you will find three (3) different portals for specific forms:
1. ILC Required forms (Medical & Code Of Conduct forms for EVERYONE attending the ILC)
2. Hotel forms (Hotel confirmation and rooming list)
3. Chaperone Form – If your student(s) are attending ILC without their advisor.
You do NOT need to submit any forms via the National HOSA website.
1. Conference Forms/Uploads
All forms should be submitted electronically below.

Before submitting, you should ensure:
1. To separate the forms into two categories
a. Code Of Conduct Forms (upload in the Code of Conduct link below, PDF Files ONLY)
b. Medical Forms (upload in the Medical Form link below, PDF Files ONLY)
2. Ensure all forms are organized alphabetically by last name. Include all advisors, chaperones/family, and students together.
3. Every form must include ALL signatures. NO ELECTRONIC SIGNATURES will be accepted.
4. Submit your forms only after you have gathered all individuals who are registered through your chapter.
5. EVERYONE registered for the conference MUST have a Medical form and Code of Conduct form
6. All forms will be reviewed for signatures. Should any forms be incomplete or lack signatures, the forms will be returned to you as incomplete, and you will need to resubmit.
7. All Submissions should be in PDF format ONLY.
Advisors/Chaperones are required to carry one printed copy of all forms with them during the conference. Paper copies will not be submitted at SC HOSA’s ILC registration.

All Schools must submit their HOTEL CONFIRMATION to SC HOSA

All Schools must submit a HOTEL ROOMING LIST to SC HOSA. For a room list template, click here. Once you have completed the rooming list. you will need to save and submit it below in a PDF document.
3. Chaperone Information
The CHAPERONE FORM is meant for students who are NOT being accompanied by their HOSA advisor. It applies to students who are traveling solely with a parent, another school, or another individual from the district, among others.
Once you’ve completed the form, you will need to save it as a PDF and upload it to the portal below.
