Deadline for form submissions - OCTOBER 3rd
All forms should be submitted electronically below.
Before submitting you should ensure:
1. To separate the forms into two categories
a. Code Of Conduct Forms (upload in the Code of Conduct link below, PDF Files ONLY)
b. Medical Forms (upload in the Medical Form link below, PDF Files ONLY)
2. Ensure all forms are organized alphabetically by last name. Include all advisors, chaperones/family, and students together.
3. Every form must include ALL signatures. NO ELECTRONIC SIGNATURES will be accepted.
4. Submit your forms only after you have collected everyone who is registered through your chapter.
5. EVERYONE registered for the conference must have a Medical form and Code of Conduct form
6. All forms will be reviewed for signatures; all forms have been submitted. If any forms are missing, your forms will be returned as incomplete, and you will need to resubmit.
Advisors/chaperones are required to carry one set of printed copies with them during the conference.
Please note that you will NOT be submitting any paper copies at registration.

PLEASE REMEMBER:
- EVERY attending Advisor, Student, Guest, and Chaperone must have a medical form.
- EVERY attending Advisor, Student, Guest, and Chaperone must have a code of conduct form.
Submit all Code of Conduct and Medical Forms- All signatures are required and must be on each form.
