SUBMISSION INSTRUCTIONS:

Please ensure that all forms are organized alphabetically by last name, with advisors, chaperones, and students listed together. When scanning, divide the forms into two sets: one for Code of Conduct forms and the other for Medical forms. Please send COMPLETE sets.

• Student Code of Conduct
• Advisor/Guest/Chaperone Code of Conduct
• Medical Form (EVERYONE attending; STUDENTS, ADVISORS, GUEST & CHAPERONE)

PLEASE REMEMBER:

• EVERY attending Advisor, Student, Guest, and Chaperone must have a medical form.

• EVERY attending Advisor, Student, Guest, and Chaperone must have a code of conduct form.
