All forms should be submitted electronically below.  

Before submitting you should ensure:

1. To separate the forms into two categories

          a. Code Of Conduct Forms (upload in the Code of Conduct link below, PDF Files ONLY)

          b. Medical Forms (upload in the Medical Form link below, PDF Files ONLY)

2. Ensure all forms are organized alphabetically by last name. Include all advisors, chaperones/family, and students together.

3. Every form must include ALL signatures. NO ELECTRONIC SIGNATURES will be accepted.

4. Submit your forms only after you have collected every one that is registered through your chapter. 

5. EVERYONE registered for the conference must have a Medical form and Code of Conduct form

6. All forms will be checked for all signatures and ensure you have all forms.  They will be sent back to you if there are incomplete forms.

7. You do NOT need to submit your Code of Coduct and Medical forms through the HOSA National link. Once SC HOSA has all forms, we will submit all forms as a State.

• Advisor/Guest/Chaperone Code of Conduct (Use this form for all advisors, guests, family and chaperones)

• Medical Form (EVERYONE attending; STUDENTS, ADVISORS, GUEST & CHAPERONE)

• Your Chapter’s Rooming List- Once you have completed the rooming list sheet, you will need to convert it to a PDF to upload in the below form

• Your Hotel Confirmation

• Chaperone Form (submitted below in the form)

  • If an advisor from your current HOSA chapter is unable to accompany their chapter’s students, a chaperone form must be filled out (all required signatures must be included; electronic signatures will NOT be accepted). If only a parent is attending with the student, they must also complete the form with all necessary signatures. Additionally, the parent must be registered for the conference.

PLEASE REMEMBER:

• EVERY attending Advisor, Student, Guest, and Chaperone must have a medical form.

• EVERY attending Advisor, Student, Guest, and Chaperone must have a code of conduct form.
